Form creation form

No, the title of this is not wrong. This is a form to help non-developer-people build forms. See attachment for complete form.

We sat down with the back end developers at work and assessed all the information they are going to need to build out these forms, and then my design manager and I drew iteration after iteration of this form until it reached its cleanest and most efficient form (see whiteboard scribbles). Not sexy, but gets the job done :)
