List - blank state, partial, and finished

I've been struggling with clearly communicating the intent to users when they move to this tab (part of a multi-step form). The intent is to clearly convey that they need to enter 2 years of residence history, and cannot continue with the form until they have provided those 2 years of history.
I've spent several days trying different interfaces, so I think I've gotten very biased towards my work and need some fresh ideas to provide some helpful criticism.
Some things I'm specifically looking for feedback on:
(1) On the blank state I have "Residence History" centered (1st attachment), but after the user starts adding records I move that title and the green Add button to the left (2nd attachment).
(2) Is the location for the messages good? ("add additional residences", "you have entered 2 years...continue").
The first attachment is the initial "blank" state. The 2nd attachment is after the user has added a record but we still need additional data. The 3rd attachment is once they have provided enough data and are ready to continue.
