Naveon: Care facility web application

Naveon’s Post-Acute Care apps help Patients and their Care Teams get to know each other, have important (and sometimes difficult) discussions, and stay connected even after a Patient is discharged.

The web app is where a care facility’s administrators customize the app experience for their patients. They can pull in data from a patient's electronic medical record, track the services or types of care a patient is receiving based on their diagnosis, prepare a discharge summary the patient can see in their mobile app, and define and manage many other aspects of a patient's plan of care.
