Apply For Leave Dashboard Design

Hello Dribblers,

I designed an "Apply for leave" UI recently which was fairly simple. I just kept in mind the end users of the application who are in a hurry when they are using this functionality (because who doesn't like a vacation!). So only the most necessary input elements exists here. What do you think about this UI?
