SRXP Expenses App - Mileage form

Hi, i've been slaving away on a expenses app for SRXP. Specifically the forms. And these can be very boring, because well they are forms.

This screen here is the second step of filling in the form, the part where you enter in the details and submit the form. I've tried to make it more visually appealing but I hit a brick wall now. I attached the full screen so that you can analyse it.

Any tips/suggestions?
