Passport form / 2021

I had a paper application form for a replacement passport, 
with an example of filling out. I needed to make an electronic application form out of it.

I analyzed the existing form and divided all the fields into semantic groups – it’s helped me to break the filling out 
of the electronic form into several steps.

First, I worked on the prototypes of the service in sketches. Then I moved on to wireframes and visual design. The service should be clear and not distract the user from filling out the form.
