Interactive Form Design

This is a mockup of a web application for a Meals on Wheels website, done for class.

This application was designed for desktop, but could easily be converted to mobile with changes to page dimension and font size. In addition, one design goal was for the form to be easy to convert to a printable version. Many people receive Meals on Wheels applications from a social worker or doctor instead of seeking it out themselves. With that in mind, an emphasis was put more on fillable fields and selection boxes instead of on dropdowns.
