User Form Design

Optimized user forms are an art unto themselves. This application asked for a series of personal answers as part of a sign-up process to support delivery and distribution of COVID-19 vaccines early in 2021. Due to medical and regulatory requirements of how doses were being allocated at that time, based on an individual's age, occupation, and other risk factors, as well as geographic challenges to vaccine distribution, this form was lengthy but needed to be friendly. Logical field grouping, divider lines between sections, conditional field relationships and other considerations contributed to usability of this important form. This was produced as part of a project delivered with GovWebworks.
