I masked off Apple's new iPhone5S (Dat Gold Edition) for my own purposes and thought I'd share. Basically this is a PSD with very few layers and a masked-off zone for the screen. The shadow is in a separate group, and the phone is fully cut-out.
Total document resolution is 3250x2000px.

Thanks to @NckJrvs for making a quick Smart Object variation so it's easier for you peeps to drop in your screens. It's also included as one of the attachments.

Thanks @Ryan Ford !
Appreciate it, Ryan. It's perfect.
@Norimaki odd, I had no trouble on my end.
@Norimaki I have the same. The smart object contains an canvas that is bigger than 640x1136.
Many thanks for the iPhone mockup.
However, the smart object canvas is the wrong size (696 x 1269).
