A challenge was raised to create a digital program to help intercept potentially dangerous health conditions for at risk patients prior to emergency.
Patients were enrolled in the monitoring program and sent home with network-connected scales, blood pressure and heart rate monitors. As patients log this information daily, the assigned nurses are be able to quickly identify major changes in order to prevent emergent health scenarios and clear alerts after addressing the issues.
This solution was successfully implemented and has been a big part of saving 1 patient's life in a blood clotting emergency.
