Need help designing a simple scorecard

I'm looking for an initial small project (but may have ongoing design needs) for a reusable scorecard that I want to develop.

I have a sketched image I can send you - I think this is a fairly straightforward job and could potentially fit in between larger projects. Please let me know if you're interested - I would love to hear from you :)
