I like it. There is a bit of "too much" padding on the input fields text, less 5px to the left would do magic. :)
P.S I deleted my previous comment, so yours could be on top.
I love forms like these. Non-obtrusive and simple but doesn't seem out of place. I'd like to see more context around it though.
Left padding of the placeholder text seems a bit wonky compared to the carrot on the right of the "Interested in" drop down field.

What is your method for custom drop down menus? Looks great btw!
These kind of "plain" forms are more and more appealing to me the more people over skeumorph forms.
Simple is the most difficult thing to do. I like it!
