Overall it looks fantastic. One thing I don't like is the two circle parts of the note, at the bottom, pop out too much. It needs to be more subtle, like the rest of the note. Make that one change and I think it becomes perfect.
Oh yeah, and it would be cool if you made it in different colors, like red, green, etc. so people could customize the look of their itunes icon.

I think it's great, really good work. My only critique would be that some of the edges seeme too sharp.. causing parts of it to have some aliasing.
@Alex Cool, I'll try to tweak the note as you suggested.
@Kevin Exactly my thoughts. Funny I did notice it once uploaded to dribbble. Great way to see the work out of context and PS environment. You can spot the imperfection at once.
