I was eagerly waiting for the launch of the new iPhone 5, not just for the device per se. I also needed it for a new website/app I'm building.
So the first thing I did was grab a snap and create a handy template. You're all free to download, use it & and share as you please! All credits go to Apple.
Instructions: Edit the Screen Smart Object (1136x640 pixels). I've skewed & transformed it so it fits perfectly on the body of the iPhone 5.

So you made some cut and paste with Apples press image and now releasing it as a template? Did you read the "image use Agreement" that you agreed to when you downloaded the image? I hope you understand that it's illegal and everybody that using this is making something illegal aswell. Just telling...

Hey @Mikael Eidenberg, I'm pretty sure you're right and this could well be an "illegal" alteration of the original Apple Press Image. But, I never misguided anyone and never said that I rendered it in 3D myself. I published it just a couple of hours after the official launch of the device... I'm also aware that more than half the publishers use the same press images to promote their apps...
My purpose was to provide a solid photoshop template (smart objects, lighting & layers) for my personal use, and decided to share with the world. That's it...
