Clean. On the attachment the labels for the drop downs feel too far away. Maybe right aligning the text would make it easier to look at.

Love the padding! Agree with @Pete Lacey on ditching the avatar drop shadow. Why isn't the first dropdown an input field with autosuggest if I may ask?
