   I don't think the expiration dropdowns work as having an inner bevel. I'd make them pop as they blend in too much with the input fields.

    Can you show us how the form looks without the large logo in the background?
    It's really clear, lately I had to fill in my credit card info a lot. Compared to those crappy forms this one makes me smile!
