With this quality of UI design, it's a fucking shame you need those shitty 99designs contests. You shouldn't need it and focus on the real clients instead.
One thing though is that I'd worry that the lack of color in the sidebar icons would mean that the user would have to either read or examine it closely nearly every time they use the chart. This would increase cognition time and thus the likelihood of error, both pretty risky and important thing with a medical chart. Adding some subtle yet distinct color to those icons would help to avoid that.
One question: Is the iOS clock bar at the top from a screencap? It seems to be at a different resolution than the rest of your shot.
