Hallo Dribbblers,
Another basic form for every clinics to get patients appointment especially to collect their patient data and more details from them. This time I made it for mobile too.
It's a basic form and must get the big attention from patients, but stay simple, clean, and easy to reach is the goal.
Basic typeface and tailwind colors are used for it.
