   I love this! Nice label / field combo, form title, and down to the optional note. The Submit button in the full comp is a little confusing, but I'm guessing it makes sense if you're on the site?

   @Ted you raise a good point. Definitely was one of my concerns. When they click 'personalize' an additional text field for a message will appear beneath the last input field. The idea is that they can add more details if they need to, but otherwise they can keep it real simple. Maybe 'Add Comment' might be more appropriate.
