Naveon: Content library management

For Naveon’s Post-Acute Care mobile app, we created a personalized experience for Patients receiving post-acute care. The app helps Patients and their Care Teams get to know each other, have important (and sometimes difficult) discussions, and stay connected even after a Patient is discharged.

The web is where care facility administrators customize the app experience including what content items a patient sees based on their diagnosis. Care providers can create content items, associate them with diagnoses, and drag and drop to re-order the hierarchy of the content library. They can also recommend specific content items to individual patients.
