   Okay. I really need feedback on this.
   I feel like the form has potential but I'm a bit puzzled on what to fix.
   I'm still not happy with it.

   I really love the details on the box. :) Just bother me the gradient on the fields (maybe you can try something like this > http://cl.ly/3w0W3P3L2r3C3H3i0H2h) and the send button, something is not unified there.

   I have to agree with Joni, the send button has little to do over there.

   Apart from that, I don't know if I'd change anything. Maybe if I saw the form in context...
