Payment Capture

Streamlined payment capture flow.

Wanted the experience to be clean and simple. Subtle details like:
1. Indicating which card after the first digit is entered
2. No advance button, the MM/DD and CVC fields display after the last CC digit is entered.
3. Remember payment and Billing/Shipping info displays after last digit from CVC is entered.
4. Ability to add a different ship to address (by default, anticipates they are the same)
