@Chase Giunta I'm betting that Cuberto did most of those reflections by hand [although I, again, could be wrong]. It's very convenient to be able to trace reflections, though–the reflection of the cloud image on this icon, for example, or the reflections of the wood here were traced from 3D renders I made :)

@Chase Giunta It's probably C4D or Maya [EDIT: They use 3DS Max, I was wrong :D]. What I do [and what SoftFacade does, and probably Cuberto as well] is render a high-res flat image from the 3D program, then trace the render in vector to get all of the perspective correct. Then you do the shading in PS–although you can always go and check your lighting with the 3D model to make sure that reflections, shadows, etc. are accurate.
Making icons entirely in 3D that look like icons is really, really hard. The only one I've seen do it well is Rich Dellinger. Most of the time the best result is made in PS, using the 3D renders as reference.

@Chase Giunta, usually we draw icons 100% in Photoshop. In this case, it will be a set of three icons in the same perspective. So we have made 3d models of all three icons in one scene to avoid errors with the perspective and the light source. All the rest things were painted in Photoshop.
